Continuum of Colorado has an opportunity for an Employment Consultant/ TANF Case Manager who will be responsible for coordinating and providing integrated employment services for individuals with intellectual and developmental disabilities and Temporary Assistance for Needy Families (TANF) case management services (approximately 50/50 split).
Our Employment Consultant will assess work readiness, provide ongoing vocational training to help long-term services and will support (LTSS) individuals to complete their vocational goals. For individuals enrolled in the TANF program, our Case Manager will assess and deliver support services, education and work opportunities. In both cases, our Employment Consultant/ TANF Case Manager will focus on providing resources to help individuals address barriers and seek independence through obtaining and maintaining community based employment. 
This is a full-time position (36 hours/ week) that will be based out of our Jewell office located in Aurora, CO (exit 5 off of I-225). We’re committed to a great work-life balance here so our offices are open Monday - Thursday and are closed on Fridays. This position has a flexible schedule and will split time between a work from home environment and working face to face with our individuals in community-based settings. 
More about what you'll get to do
  • Coordinate services and supports (i.e. Division of Vocational Rehabilitation, employers, Social Security, TANF County offices, local service agencies)
  • Conduct assessments to determine barriers to employment and work readiness
  • Coordinate with individual receiving supports, family and multidisciplinary team to develop goals
  • Provide training on topics including job search skills, job keeping skills, budgeting and goal setting
  • Assure all rules and regulations, and agency policy and procedure are adhered to in service coordination, and provision of services
  • Develop Individualized Plans (IPs) based on goals for achieving self-sufficiency, and monitor progress of TANF participants
  • Communicate with service providers and health care professionals to help determine limitations, abilities and restrictions to self-sufficiency for individuals receiving TANF supports
  • Enter and maintain data in Colorado Benefits Management System (CBMS) for individuals receiving TANF supports
  • Coordinate with the Division of Vocational Rehabilitation, various County and other external entities, as needed

Requirements to be our Employment Consultant/ TANF Case Manager
  • High school diploma or GED
  • One year of case management experience, required
  • Ability to resolve conflict under pressure
  • Professionally interacts and communicates with a diverse group of individuals
  • Effectively writes, reviews, and implements plans
  • Demonstrates good organizational skills
  • Ability to work independently with minimal supervision
  • Personal vehicle in good operating condition for use during work 
  • Valid Colorado driver's license; proof of insurance; 3 years of current, consecutive driving history
  • Meet our driving requirements - no suspensions or revocations in the past 3 years. Less than 3 tickets in the past 3 years
  • Experience with Colorado Benefits Management System (CBMS)
  • Bachelor’s Degree, preferred 
  • Working knowledge of the IDD system, rules and regulations and DVR
  • Two years experience and/or related education or training working with individuals with developmental disabilities 
  • Knowledge of various vocational training techniques and basic behavior management principals
