FT - 36 hrs/week
$17.00 - $19.00

Continuum of Colorado has an opportunity for an Employment Consultant - TANF Case Manager who will be responsible for communicating, coordinating and providing integrated employment services for individuals with intellectual and developmental disabilities and Temporary Assistance for Needy Families (TANF) case management services (approximately 50/50 split). This position also coordinates with the Division of Vocational Rehabilitation, various County and other external entities as needed.

This is a full-time position that will be based out of our Inverness office located in the Park Meadows area in Englewood. We’re committed to a great work-life balance here, so our offices are open Monday - Thursday and are closed on Fridays. For this position, the shifts will vary depending on the individual in services schedule and may include some evenings and weekends.

  • Coordinate services and supports (i.e. Division of Vocational Rehabilitation, employers, Social Security, TANF County offices, local service agencies)
  • Develop and maintain current and organized case files
  • Conduct assessments to determine barriers to employment and work readiness
  • Assist individuals in overcoming barriers to employment
  • Provide training on topics including job search skills, job keeping skills, budgeting and goal setting
  • Assist consumers to obtain resources, makes referrals, and provide follow-up
  • Assure all rules and regulations, and agency policy and procedure are adhered to in service coordination, and provision of services

TANF Case Manager

  • Develop Individualized Plans (IPs) based on goals for achieving self-sufficiency, and monitor progress of TANF participants
  • Maintain contact with Count eligibility technicians and attend monthly staffing to collaborate with County staff in order to best serve TANF consumers
  • Communicate with service providers and health care professionals to help determine limitations, abilities and restrictions to self-sufficiency for individuals receiving TANF supports
  • Request supportive service payments from County to assist TANF participants in removing barriers to self sufficiency
  • Complete and maintain current all County training requirements required for TANF case management provision
  • Enter and maintain data in Colorado Benefits Management System (CBMS) for individuals receiving TANF supports, meet required deadlines

Employment Consultant

  • Coordinate with individual receiving supports, family and multidisciplinary team to develop goals
  • Monitor and assess job-related skills and provide employment skills training and follow-along support
  • Assist individuals receiving supports with employment leads and job placement
  • Facilitate natural supports for individual in services and provide consultation and education to employers
  • Advocate for equitable access to integrated employment for individuals with developmental disabilities
  • Complete Health/Safety Assessments and Incident Reports as required

Must haves to be our Employment Consultant - TANF Case Manager

  • One year of case management experience required
  • Prefer experience with Colorado Benefits Management System (CBMS)
  • Bachelor’s Degree preferred.  Additional related work experience may be substituted for advanced study requirement
  • Experience and/or related education or training working with individuals with developmental disabilities
  • Prefer working knowledge of the IDD system, rules and regulations and DVR
  • Knowledge of various vocational training techniques and basic behavior management principals
  • Ability to resolve conflict under pressure
  • Ability to modify supports to individual needs
  • Professionally interacts and communicates with a diverse group of individuals
  • Maintain necessary training's and certifications
  • Effectively writes, reviews, and implements plans
  • Personal vehicle in good operating condition for use during work
  • Ability to meet agency driving requirements and operate agency vehicles
  • Ability to perform math calculations necessary to carry out job responsibilities
  • Demonstrates good organizational skills
  • Ability to work flexible hours
