Who We Are
Continuum of Colorado is a nonprofit agency providing services to people with a wide range of abilities. We empower individuals to live a meaningful life. We believe it is our responsibility to not only meet the needs of each individual we support but also to partner with our supported individuals to help them reach milestones, achieve their goals, and make their dreams a reality. Our mission is to be a partner in choice and independence by collaborating with individuals and their families to provide high-quality lifetime supports that will empower them to thrive in their community.
What We Do
At Continuum of Colorado, we provide direct support through a variety of services including Residential Services, Day and Seniors’ Choice activities, Family Caregiver programs, Supported Employment, and Home and Community Support.
Residential Services
The Residential Services Department focuses on a person-centered approach to providing services. As a part of a holistic services approach, we encourage involvement with family and friends and are committed to addressing the total needs of each individual in a typical home environment. We have a variety of positions in our Residential department from Direct Service Professionals (aka: Life Skills Specialists (LSS)) and Family Care Givers to Residential Supervisors who oversee our group homes as well as Host Home Provider opportunities.
Supported Employment
Our Employment Consultants assist individuals with vocational training techniques and promote independence in all aspects of an individual’s job. Our Supported Employment Department’s positions offer flexibility and variety in the role and the schedule as well as having more dedicated time for the individual you’re serving.
Day & Seniors’ Choice Activities
Our Day & Senior’s Choice programs serve adults with abilities that range over a broad spectrum. Our participant to staff ratio is 1:5 and we offer activities that include both on-site and community outings with meaningful person centered activities to meet varying interests and abilities. We’re on the lookout for enthusiastic employees to fill positions such as: Activity Services Facilitator and Activity Services Lead. Being a CNA is a plus as well!
Home & Community Support
This program provides a variety of quality services to adults and children by our trustworthy and caring providers. Many times the families and individuals we serve choose their own providers via family members and family referred applicants for our Personal Care Worker roles so they can manage the direction of their services. In addition we also have positions for Massage Therapists, Family Care Givers, Movement/Music Therapists and Personal Care Supervisors.
